What We Collect

This poll collects only your answers to the 10 multiple-choice questions. Nothing more. Specifically, we record:

We do not collect your name, email address, phone number, home address, IP address, device type, or any other personally identifiable information.

Who Can See Your Responses

Only authorized internal researchers reviewing aggregate community data have access to poll results. Results are analyzed as totals and percentages โ€” for example, "42% of respondents travel more than 25 miles for care" โ€” never as individual records.

Your individual responses are never reviewed, singled out, or linked back to you in any way.

How Your Data Is Used

Poll responses are used solely to better understand healthcare needs, preferences, and access challenges across Southern Ohio communities, including Ross, Pickaway, Fayette, Highland, Hocking, Pike, Jackson, and Vinton counties.

Results may be summarized in internal reports or presentations to inform community health planning and outreach decisions.
